The ghrelin receptor, and what else it does
GHRP-6 is a six-residue synthetic peptide acting at the ghrelin receptor, one of the first compounds to establish that this receptor could be used to drive growth hormone release at all.
Ghrelin is also the hormone that signals hunger. Agonism at its receptor does both jobs, and with GHRP-6 the appetite effect is pronounced enough to be its defining characteristic in practice.
There are two separate doorways into growth hormone release, and this compound uses the second one. Why selectivity became the goal
Along with appetite, GHRP-6 raises cortisol and prolactin. Between them, these three effects meant the compound moved several hormonal systems when the intention was to move one.
Ipamorelin came out of the effort to fix that. It reaches the same receptor and was designed to produce the growth hormone response without the appetite, cortisol and prolactin effects that characterise the earlier compounds.
The appetite effect is a methodological obstacle as well as a practical one. In an animal study where intake is not controlled, a compound that drives feeding will move body composition by that route as well as by any hormonal one, and telling the two apart takes deliberate design rather than careful wording afterwards.
